Who we are
Pacific Rehab Consultants ("PRC", "we", "us" or "our") is a physician owned and operated physiatry group that provides rehabilitation medicine to skilled nursing and assisted living facilities across California and the Pacific Northwest. This Privacy Policy explains how we collect, use, share and protect personal information when you visit pacificrehabconsultants.com (the "Site") or contact us through it.
This policy covers the Site only. Information about how we handle the medical records of the patients we treat is in our HIPAA Notice of Privacy Practices.
Information we collect
Information you give us. When you contact us by email or phone, or book a call through the Site, we collect the details you enter, which may include your name, email address, phone number, company or facility name, the type of inquiry, and your message.
Scheduling. Links on the Site take you to Calendly, a third party scheduling service, to book an introduction call with our team. Calendly collects the information you provide on its pages, such as your name, email address and the time you choose, and shares the booking details with us. Calendly's own privacy policy governs its collection and use of that information.
Career inquiries. Physicians, physician assistants and nurse practitioners interested in joining PRC schedule a call with us through Calendly. The Site does not accept resumes, CVs or other uploaded documents. Anything you choose to share on a call or by email afterwards is handled as part of our recruiting process.
Information collected automatically. Like most websites, the Site and the analytics tools it may use collect certain technical information automatically, such as your IP address, browser type, device type, operating system, the pages you view, the time and length of your visit, and the website that referred you. See the Cookies and analytics section below.
Protected health information
The Site is not a patient portal. It has no login, accepts no payments, and is not designed to collect protected health information as defined under the Health Insurance Portability and Accountability Act (HIPAA). Please do not submit medical records, diagnoses, insurance details or other health information about yourself or anyone else when you contact us. If you are a patient, family member or facility with a question about care, call us using the phone numbers below.
How we use information
We use the information we collect to:
- Respond to your inquiry and communicate with you about our services.
- Arrange and conduct introduction calls with facilities and clinicians.
- Evaluate and communicate with prospective clinicians about opportunities with PRC.
- Operate, maintain, secure and improve the Site.
- Understand how visitors use the Site so we can make it more useful.
- Comply with legal obligations and protect our rights, our visitors and the public.
We do not use the Site to make automated decisions about you, and we do not use information collected through the Site for targeted advertising.
Cookies and analytics
The Site may use cookies and similar technologies, and may use web analytics services such as Google Analytics, to measure traffic and understand how the Site is used. These tools may set cookies in your browser and collect the technical information described above. Analytics data is reported to us in aggregate form and is used only to improve the Site.
You can control cookies through your browser settings, including blocking or deleting them. Google also offers a browser add-on that opts you out of Google Analytics measurement. Some parts of the Site may not work as intended if cookies are disabled. The Site does not currently respond to browser "Do Not Track" signals.
How we share information
We do not sell personal information, and we do not share it with third parties for their own marketing. We share information only in the following circumstances:
- Service providers. With companies that help us operate the Site and our business, such as our website hosting and form processing providers, Calendly for scheduling, and analytics providers. These providers may use your information only to perform services for us.
- Legal requirements. When we believe disclosure is required by law, regulation, subpoena or court order, or is necessary to protect the rights, property or safety of PRC, our visitors or others.
- Business transfers. In connection with a merger, acquisition, financing or sale of all or part of our business, in which case the information may be transferred to the successor subject to this policy.
- With your consent. When you ask us to share information, or otherwise agree to it.
Your California privacy rights
If you are a California resident, California law may give you rights over the personal information we hold about you, including the right to know what personal information we have collected and how we use and share it, the right to request that we delete it, the right to request that we correct inaccurate information, and the right not to be discriminated against for exercising these rights. Because we do not sell or share personal information for cross-context behavioral advertising, there is no need to opt out of those activities.
To make a request, email us at [email protected] or write to the address below. We will need to verify your identity before acting on a request, and we may ask for information to do so. You may also designate an authorized agent to make a request on your behalf.
California's "Shine the Light" law lets California residents ask about the disclosure of personal information to third parties for their direct marketing purposes. We do not disclose personal information for that purpose.
Data retention
We keep messages you send us and related correspondence for as long as needed to respond to you and to maintain a record of our business relationship, and then for as long as required by law or reasonably needed to resolve disputes and enforce our agreements. Analytics data is retained according to the settings of the analytics provider.
Security
We use reasonable administrative, technical and physical safeguards designed to protect the information we collect through the Site. No method of transmission over the internet or method of electronic storage is completely secure, so we cannot guarantee absolute security. Please do not send sensitive information when you contact us.
Children
The Site is intended for adults, including facility administrators and healthcare professionals. We do not knowingly collect personal information from children under 13. If you believe a child has provided personal information through the Site, contact us and we will delete it.
Links to other websites
The Site links to websites and services we do not operate, including Calendly. We are not responsible for the privacy practices or content of those sites, and we encourage you to read their privacy policies.
Changes to this policy
We may update this Privacy Policy from time to time. When we do, we will post the revised policy on this page and change the "Last updated" date above. Your continued use of the Site after a change means you accept the updated policy.
