Our STORY

Where we came from...

Pacific Rehab Consultants (PRC) is a physician-owned rehabilitation company providing PM&R specialty trained providers to best support the needs of post-acute care facilities. Dr. Michael Decker, D.O. founded PRC to improve & optimize care delivery through communicative and educational collaboration within each facility.

With our experience in a multitude of acute and post-acute care settings, we are able to take this deep knowledge and patient-centered approach and apply it within the confines of the skilled setting to improve our patients’ outcomes and the performance of each facility. One of our primary goals is to create a cohesive network within each facility by working directly with primary physicians, rehabilitation therapy and staff, ancillary staff, nursing, social services and the administration within the facility to create a truly interdisciplinary approach toward optimal patient care including reducing hospital readmissions and recognizing and treating preventable medical complications.

Physical Medicine and Rehabilitation (PM&R) is a specialty with a musculoskeletal and neurologic focus within the framework of promoting rehabilitation of the physical body to the highest level.

As a board-certified Physical Medicine and Rehabilitation physician, Dr. Decker specializes in post-acute rehabilitation and has a long, fruitful track record of working directly with facilities to optimize each patient’s experience and is seen as an essential member of the post-acute team. Given his experience in the post-acute setting and internal workings of each facility, his expertise is frequently utilized on a consultation basis to provide facility recommendations and specific oversight regarding clinical and/or other operations. He has personally trained many colleagues and each of his employees to understand the dynamics of these facilities and how to provide patient care and meeting the unique needs of each and every facility.

Our CORE Values

The values that shape everything we do

Service to Others

How we show up for our patients, our communities, and our families.

Take Initiative

We actively look for and find problems to solve, then we solve them.

Act with Integrity

We do what we say we’re going to do with regards to the patients and facilities. It not only helps the patients to build confidence in us, but we also build confidence in ourselves.

Be Present

We are where our feet are. We show up, we do what we say we are going to do, and we take care of any issues in front of us.

Always Be Learning

We strive to continue to grow, not only from an educational standpoint, but in all the things we don’t know.

Be Humble

Humility is the one of the cornerstones to being a physician. When we’re with patients, we don’t always have to know the right answers, but we have to express to them that we want what's best for them and we will find out the answers.

Details Matter

Some of the smallest details mean the most not only to the patients and facilities, but also their outcomes.

Take Ownership

We go above and beyond and we accept not only patient responsibilities, but also the outcomes that the patients and facilities have.

Be Grateful

We really appreciate where we are in our lives and the interactions we get to have with our patients.

Give Generously

This is what we do when we’re at the facilities, it’s what we do in our lives, it is how we interact with our world. Because we have so much, we do our best to give back.
