Pacific Rehab Consultants is a 100% physician-owned physiatry group founded by Dr. Michael Decker, D.O. with one focus: build a rehabilitation company where clinical communication, education, and teamwork are the standard. We provide physiatry care in skilled nursing facilities across the West Coast and we are growing fast.
The Schedule
Monday through Friday. No evenings, no weekends, no on-call. You focus on patients and we handle the rest.
Compensation
This is a 1099 independent contractor position paying 70% of collections. You set your pace and your earnings scale with your work.

Responsibilities
You will lead comprehensive PM&R evaluations and follow-up care at skilled nursing facilities. Day-to-day responsibilities include:
- Conducting PM&R consultations and follow-up visits
- Prescribing medications and ordering and interpreting diagnostics
- Developing and overseeing personalized care plans
- Supervising and mentoring our PAs and NPs
- Participating in interdisciplinary team rounds
Job requirements
- MD or DO with an active, unrestricted California medical license
- Board certified or board eligible in Physical Medicine & Rehabilitation (PM&R)
- Active DEA registration
- Post-acute or SNF-based experience preferred
